(524) Risk Factors for Readmission With Delayed Postpartum Preeclampsia: A Retrospective Case-Control Study

Objective:
To identify patient characteristics and risk factors for readmission for delayed postpartum preeclampsia (DPP).

Study Design:
We conducted a retrospective cohort study of patients who delivered at Montefiore Medical Center between June 2018-May 2022. Cases of DPP, defined as new-onset preeclampsia diagnosed during a postpartum readmission between 2 days and 6 weeks after delivery in patients without a prior hypertensive disorder of pregnancy (HDP), were identified via ICD-9/10 codes and verified by chart review. A random sample of 400 patients without HDP and without readmission was selected from the same cohort. Bivariate tests compared baseline characteristics. Multivariable logistic regression assessed factors associated with DPP readmission, adjusting for age, race, body mass index (BMI), insurance status, delivery mode, and delivery timing in relation to the COVID-19 pandemic.

Results:
Of 17,711 deliveries, 114 patients (0.64%) were readmitted with DPP. Compared to controls, patients with DPP were more likely to be older (36.9 ± 5.8 vs. 33.3 ± 5.5 years, p</span> < 0.01), identify as Black (47.4% vs. 27.8%, p < 0.01), undergo cesarean section (48.2% vs. 31.7%, p < 0.01), and deliver post-pandemic (29.8% vs. 18.8% p = 0.04) . In adjusted analyses, age ≥ 35 years, (aOR 3.79, 95% CI 2.25 - 6.38, p < 0.01), Black race (aOR 2.19, 95% CI 1.35-3.56, p</span> < 0.01), and post-pandemic delivery (aOR 2.58 95% CI 1.40-4.77, p</span> < 0.01) were significantly associated with readmission for DPP. BMI, insurance status and delivery mode were not associated with DPP readmission.

Conclusion:
Older age, Black race, and post-pandemic delivery were independently associated with DPP readmission. Findings highlight the need for targeted discharge education and follow-up. The post-pandemic rise in DPP readmissions may reflect improved recognition of DPP due to shifts in care delivery in response to the COVID-19 pandemic.
