Objective:
To evaluate the impact of low-dose aspirin recommendations on the rates of gestational hypertension and preterm birth, and to assess the association between gestational hypertension and preterm birth over time.

Study Design:
A retrospective cohort analysis was conducted using CDC Natality data from 2011 to 2024. Multinomial logistic regression assessed the association between gestational hypertension and preterm birth rates over time. Adjusted odds ratios (aOR) with 95% confidence intervals (CIs) were calculated, with statistical significance set at p< 0.05. The analysis controlled for maternal BMI, age, in-vitro fertilization (IVF), race, insurance type, gestational and pre-gestational diabetes, hypertensive disorders of pregnancy, eclampsia, and inter-pregnancy interval.

Results:
Rates of gestational hypertension increased across low, moderate, and high-risk groups during the study period. Additionally, rates of preterm birth in moderate and high-risk groups decreased during the study period. However, adjusted odds ratios (aOR) for preterm birth among individuals with gestational hypertension demonstrated a steady decline from 2.824 in 2011 to 2.208 in 2024 (Table 1). Among high-risk patients specifically, the decline was most pronounced and steadily decreased over time (Figure 1).

Conclusion:
Following the USPSTF recommendation for aspirin administration to prevent early-onset preeclampsia in 2014, preterm birth rates among patients with gestational hypertension have declined. This trend may indicate the efficacy of low dose aspirin on prevention of early onset gestational hypertension.
