Objective:
Sociostructural barriers yield disparate perinatal outcomes. Identifying broad policies that reduce health disparities can inform targeted interventions. The Social Vulnerability Index (SVI) quantifies neighborhood-level sociostructural adversity. We examined if expanding access to elective inductions of labor (IOL) post-ARRIVE trial changed SVI-based perinatal disparities.

Study Design:
This was a retrospective study of all nulliparous patients with term, singleton, vertex gestations who delivered at a large urban US hospital from January 2013—December 2022. We excluded patients with scheduled cesarean deliveries (CD). Data were abstracted from electronic medical records. Addresses were geocoded to 2020 US Census tracts and SVI values (0-1). SVI was dichotomized as high (≥ 0.75) or not high (< 0.75). The primary outcome was IOL. Secondary outcomes were CD, chorioamnionitis, and NICU admission. Our institution’s policy change allowed elective IOL for everyone ≥ 39 weeks’ gestation. A difference-in-differences analysis compared SVI-based disparities in pre-change (2013-2017) and post-change (2018-2022) cohorts. Adjusted, clustered linear probability models estimated absolute differences in likelihood, comparing (1) all individuals pre- and post-change, and (2) an interaction term representing the pre/post change disparity.

Results:
We included 21,854 patients; 22% had high SVI. Patients with high SVI disproportionately identified as non-White, lacked private insurance, and had greater rates of hypertension and NICU admission (p < 0.001). IOL rates did not differ by SVI (Table 1). IOL was more likely in the post-change period (9.0[7.5,10.5]). CD (-3.8[-5.2,-2.4]) and NICU admission (-1.5[-2.4,-0.7]) were less likely in the post-change period. The disparity in chorioamnionitis decreased post-change (-0.5[-0.9,-0.02]; Table 2).

Conclusion:
Rates of CD and NICU admission decreased post-policy change. However, SVI-based disparities were mostly unchanged. To meaningfully close perinatal health disparities, policies should target sociostructural barriers.
