This course is designed to bring together maternal-fetal medicine specialists interested in developing a robust knowledge and practice base for performing and interpreting first trimester detailed sonographic anatomic assessments of the fetus and placenta. National and International practice parameters for performance of this examination will be presented along with a variety of organ system-based lectures which will include examples of anomalies that should ”almost always” or “may be detected” in this gestational age window. Additional lectures on optimizing image quality, natural history of some anomalies and potential artifacts along with recommended diagnostic genetic assessment will be included. The critical nature of an early anatomic examination by ultrasound in the latter part of the first trimester will be discussed including health disparities, barriers to implementation and patient autonomy in reproductive choice options.

Learning Objectives:

  • Recognize differences between standard and detailed early anatomic imaging protocols, both nationally and internationally as well as the importance of adhering to them.
  • Demonstrate the ability to utilize ultrasound systems to optimize image quality to enhance detection of anomalies.
  • Interpret structural anomalies that can be identified between 11-13 weeks 6 days by utilizing 2D, 3D, color/power directional Doppler.
  • Appreciate the natural history of anomaly detection in the first trimester.
  • Justify evidence based genetic testing to findings on sonogram including an isolated enlarged nuchal translucency and anomalies.
  • Express patient perspective and autonomy for reproductive choices.
  • Analyze barriers to implementation and equitable availability of resources.
