(474) Reducing Postpartum Hemorrhage and Blood Transfusion Rates Through Standardized Anemia Management: A Quality Improvement Initiative

Objective:
Iron deficiency anemia (IDA) is common in pregnancy due to increased iron demands and is linked to postpartum hemorrhage (PPH), transfusion, and neonatal complications. This quality improvement (QI) initiative aimed to reduce PPH and transfusion through standardized antenatal anemia management— including ferritin screening, oral/IV iron therapy, and structured education—at a single academic center.

Study Design:
This single-site interdisciplinary QI project was implemented at two NYU Langone Hospital—Long Island (NYUG-LI) outpatient practices, targeting pregnant patients with hemoglobin < 11 g/dL in the third trimester. The intervention included ferritin screening at 24–32 weeks, oral or IV iron supplementation, and structured provider-patient education. Outcomes—admission anemia (Hgb < 10 g/dL), PPH (≥ 500 mL vaginal or ≥ 1,000 mL cesarean), and transfusion—were compared between focus patients receiving protocol-based care and all other NYUG-LI deliveries from July 2024 to June 2025. Stratified analysis by race/ethnicity explored potential disparities.

Results:
Among 905 focus site deliveries, 54 patients received standardized anemia management. Compared to 4,791 total NYUG-LI deliveries, focus patients had lower rates of anemia (4.4% vs. 9.2%), PPH (0.8% vs. 1.0%), and transfusion (0.7% vs. 1.1%). Figure 1 shows favorable trends over time. Table 1 highlights improved outcomes across racial/ethnic groups, with notable benefit among Black and Hispanic patients.

Conclusion:
Standardized antenatal anemia management was associated with improved maternal outcomes and may help reduce disparities in obstetric complications. Future efforts will focus on expanding the protocol across additional clinical sites, enhancing adherence to IV iron guidelines, and engaging community partners to ensure equitable implementation and sustained impact.
