Objective:
Single umbilical artery (SUA), the most common umbilical cord abnormality, is often associated with congenital anomalies, including cardiac malformations. However, data on the long-term cardiovascular impact of isolated SUA are limited. This study assessed whether isolated SUA is linked to an increased risk of childhood cardiovascular disease.

Study Design:
A population-based cohort study evaluated the risk of childhood cardiac disease (up to age 18) among infants with isolated SUA, born between 1991 and 2021 at a single medical center. Cases with fetal malformations or genetic abnormalities were excluded. Offspring cardiovascular morbidity was identified from clinic and hospital records and included decongestive heart failure, structural valvular disease, hypertension, arrhythmia, ischemic heart disease, peri-myocarditis, rheumatic fever, pulmonary heart disease, and non-specified disorders. A Kaplan-Meier survival curve was used to compare the cumulative incidence of cardiovascular disorders between both study groups; a Cox proportional hazards model was applied to adjust for potential and relevant confounders.

Results:
During the study period, 232,476 children were included, of whom 136 were diagnosed with isolated SUA. The overall cardiovascular morbidity was similar between children with or without SUA (Table). Similarly, the Kaplan-Meier survival analysis showed comparable incidence in cumulative cardiovascular morbidity during the follow up time (Figure). The Cox proportional hazards model adjusted for maternal and gestational age, diabetes mellitus, hypertensive disorders, intrauterine growth restriction and cesarean delivery, reassured that isolated SUA was not associated with an increased risk for cardiovascular morbidity of the offspring (table).

Conclusion:
In this large population-based cohort with long-term follow-up, isolated SUA was not associated with an increased risk of cardiovascular morbidity in offspring up to 18 years, offering reassurance for clinicians and parents regarding the long-term cardiac prognosis of infants born with isolated SUA.
