Objective:
Novel continuous cuffless blood pressure monitoring (CCBPM) devices have recently emerged as an alternative for traditional cuff-based non-invasive BP monitoring, but their performance in pregnancy remains unknown. Hilo (Aktiia SA, Neuchâtel, Switzerland) is a CCBPM band that estimates BP using photoplethysmography signals collected with optical sensors at the wrist. This pilot study evaluated the agreement between Hilo band BP readings and standard at-home BP cuff measurements across varying gestational ages.

Study Design:
We conducted a prospective observational pilot study of 12 low-risk pregnant and postpartum patients (3 per trimester and postpartum). This represents off-label use of the Hilo band, and Aktiia granted authorization for publication. Participants wore the Hilo band for 7 days while also performing 21 standard cuff measurements (3 per day). CCBPM readings within 30–60 minutes of cuff readings were paired. Daily average systolic and diastolic BP values between the two devices were compared using linear mixed-effects models. Agreement was assessed by calculating the proportion of paired measurements within 5, 10, and 15 mmHg.

Results:
A total of 74 average daily BP readings were obtained, including 145 paired measurements within 30 minutes and 180 within 60 minutes of cuff readings. The mean age of participants was 31.2 ± 4.1 years, and the mean body mass index (BMI) was 29.2 ± 7.3 kg/m². Fewer than 50% of systolic and diastolic readings fell within 5 mmHg; 60–70% were within 10 mmHg, and 80–85% within 15 mmHg. There was no significant difference in daily average systolic BP between devices (4.09 mmHg difference, P=0.10). Daily average diastolic BP was 5.3 mmHg lower with Hilo (P=0.02). Adjusting for BMI dichotomized at 30 kg/m² did not alter these findings.

Conclusion:
This limited dataset provides a novel assessment of a CCBPM device in pregnancy. While preliminary findings are encouraging, cuff-based devices should remain the standard of care until larger studies confirm the accuracy and reliability of cuffless technologies.
