Objective:
This study aimed to evaluate trends in breast cancer risk among pregnant women and examine factors contributing to changes in breast cancer (BC) risk over time.

Study Design:
We conducted a serial cross sectional study using data from the Nationwide Inpatient Sample. Women aged 15-54 years who gave birth from 2006-2022 without a history of BC were included. The primary outcome was BC during the delivery hospitalization, identified using ICD9 and 10 diagnosis codes. Joint Point regression was used to evaluate temporal trends in BC, and logistic regression models were employed to identify factors associated with BC risks. Multivariate logistic regression decomposition was applied to quantify the contribution of predictors to the difference in breast cancer risk between 2006-2008 and 2020-2022.

Results:
A total of 13 million births were identified. BC risk increased from 6.5 per 100,000 births in 2006 to 12.3 in 2022, with an average annual percentage change of 5.1% (95%CI: 3.9-6.4, P< 0.0001) (Figure). Demographic and clinical characteristics of delivery hospitalizations with and without breast cancer are shown in the Table. Older maternal age, chronic hypertension, family history of breast cancer, and genetic susceptibility were positively associated with BC risk during pregnancy. From 2006 to 2022, there were significant increases in the proportion of women with older maternal age and more chronic conditions. Multivariate logistic regression decomposition analysis indicated that the difference in BC risk between 2020-2022 and 2006-2008 were attributable to changes in population characteristics (55.9%) and changes in coefficients (44.1%). Equalizing the age distribution in 2020-2022 to match that in 2006-2008 would be expected to reduce BC risk in 2020-2022 by 57%. Chronic conditions contributed little to BC risk.

Conclusion:

Breast cancer risk among pregnancy population has increased over time, with advanced maternal age accounting for more than half of the increase in breast cancer occurrence.
