Objective:
Reproductive years represent a critical period in women’s lives, during which the body undergoes substantial physiological and hormonal changes that may provide insight into future health status. Concerns have been raised regarding the potential association between parity and the development of metabolic syndrome and insulin resistance in women. This study aimed to investigate the association between parity and subsequent maternal endocrine morbidity.

Study Design:
We conducted a population-based study of women who delivered between 1991-2021 at a tertiary medical center, with varying parity. We included women who had completed their reproductive period, allowing for definitive assessment of parity, and who had no known prior endocrine morbidity. Long-term maternal endocrine morbidity was diagnosed in outpatient visits or hospitalization records and was compared between parity groups. Cumulative incidence was evaluated using Kaplan-Meier survival curves, and a Cox proportional hazards model was used to adjust for potential confounders.

Results:
The study included 53,794 women who had concluded childbearing. Increasing parity was associated with an increased risk of overall endocrine morbidity, including thyroid disorders, diabetes mellitus, hyperlipidemia, and obesity (Table). Kaplan-Meier survival analysis revealed a significantly higher cumulative incidence of overall endocrine morbidity over time with increasing parity (Figure). In a Cox proportional hazards model adjusted for ethnicity and use of fertility treatments, parity remained independently associated with an increased risk of future maternal endocrine morbidity, demonstrating a dose-response relationship (Table).

Conclusion:
Higher parity is independently associated with an increased risk of maternal endocrine morbidity later in life, demonstrating a clear dose-response relationship. These findings suggest that parity may serve as a marker for future endocrine health and highlight the importance of long-term monitoring and preventive strategies for multiparous women.
