(283) Pre-Pregnancy GLP-1 Exposure and Interval Development of Gestational Diabetes During Pregnancy

Objective:
One in 8 US adults have used GLP-1 receptor agonists, with at least half using specifically for weight loss. Preconception GLP-1 use and its impact on pregnancy remains to be well described. We aim to investigate whether preconception use of GLP-1 receptor agonists is associated with an increased risk of developing gestational diabetes (GDM).

Study Design:
We performed a retrospective cohort study of singleton pregnancies receiving prenatal care and delivering at a large regional health system from 2022-2024. Those with documented pregestational diabetes, prior bariatric surgery and multifetal pregnancies were excluded. Those with documented GLP-1 use in the 12 months prior to pregnancy were considered exposed, while those without were considered controls. The primary outcome was diagnosis of GDM. Baseline characteristics were compared with chi square or Mann Whitney U tests. Regression analysis controlling for age and baseline body mass index (BMI) generated adjusted odds ratios with 95% confidence intervals.

Results:
Of 24,403 eligible pregnancies, 165 were exposed to GLP-1 receptor agonists prior to pregnancy, while 24,238 had no documented exposure. Patients exposed to GLP-1 receptor agonists had higher age, BMI and chronic hypertension, while controls were more likely to be Black and to use Medicaid (Table). GDM occurred in 22.4% of the GLP-1 receptor agonists group compared to 9.1% in the control group (aOR 2.6, 95% CI 1.8-3.8, Figure).

Conclusion:
GLP-1 receptor agonist exposure prior to pregnancy was associated with an increased odds for GDM when controlling for age and BMI.
