Objective:
There is limited evidence supporting the use of latency antibiotics < 22 weeks gestation age (WGA). Our study aimed to evaluate whether latency antibiotic administration < 22 WGA in patients with previable prelabor rupture of membranes (pPROM) is associated with a difference in maternal outcomes and prolonged latency.

Study Design:
After Institutional approval, we conducted a retrospective cohort study of pregnant individuals presenting with pPROM < 22 WGA at three tertiary care hospitals in Texas between 01/2018-03/2023. ICD-10 codes were used for chart reviews. Exclusion criteria included presentation in preterm labor, fetal demise, higher-order multiples (≥3), elective termination, or missing delivery data. Antibiotic administration, including type and duration, was abstracted from the medical record. The primary outcome was a composite of maternal morbidity, including sepsis, ICU admission and blood transfusion.The secondary outcome was latency to delivery. Baseline characteristics were compared using t-tests or Wilcoxon rank-sum for continuous, and chi-square or Fisher’s exact tests for categorical variables. Adjusted relative risks (aRR) with 95% confidence intervals (CI) were estimated using Poisson regression.

Results:
During the study period, 221 individuals met inclusion criteria, 76 (34%) received latency antibiotics. Baseline characteristics were largely similar between groups, though patients receiving antibiotics had later GA at rupture (p=0.02). There were no significant differences in the primary composite maternal outcome between the antibiotic and no-antibiotic groups (24% vs 26%; aRR 0.93, 95% CI 0.56–1.52). Antibiotic use was associated with longer latency ( 12 vs 5, p< 0.001), higher GA at delivery (median 147 vs. 131 days, p < 0.001), and greater live neonatal discharge (9.9% vs 1.4%, p=0.007; aRR 5.55, 95% CI 1.30–23.71).

Conclusion:
In pregnancies complicated by pPROM, latency antibiotics administration < 22 WGA was not associated with a reduction in composite maternal morbidity. However, it was linked to prolonged latency, higher GA at delivery, and increased neonatal survival.
