Objective:
To compare the odds of delivering a small for gestational age (SGA) neonate in pregnancies complicated by resolved versus persistent fetal growth restriction (FGR), relative to pregnancies with appropriate for gestational age (AGA) fetuses.

Study Design:
This was a retrospective cohort study of singleton, non-anomalous pregnancies diagnosed with FGR between 2017 and 2023. FGR was defined as estimated fetal weight (EFW) and/or abdominal circumference (AC) < 10^th percentile for gestational age. Resolved FGR was defined as a prior diagnosis of FGR with a subsequent EFW and AC ≥ 10^th percentile before delivery. Persistent FGR was defined as continued EFW and/or AC < 10^th percentile through delivery. SGA was defined as birthweight < 10^th percentile for gestational age at birth. Multivariable logistic regression was used to estimate adjusted odds ratios (aOR) and 95% confidence intervals (CI). Analyses were performed using SPSS v30.

Results:
A total of 9923 singleton pregnancies were reviewed and stratified into three groups: AGA (N=8858), resolved FGR (N=341), and persistent FGR (N=724). Resolved FGR was associated with over a threefold increased odds of SGA (aOR 3.44; 95% CI: 2.59–4.56) relative to pregnancies without FGR. Persistent FGR conferred the highest risk (aOR 14.83; 95% CI: 12.51–17.58). Maternal factors independently associated with SGA included substance use (aOR 1.52; 95% CI: 1.16–2.00) and a history of FGR in a prior pregnancy (aOR 2.80; 95% CI: 1.48–5.29).

Conclusion:
In our ultrasound cohort, both resolved and persistent FGR were independently associated with significantly increased odds of delivering a SGA infant, with persistent FGR conferring the highest risk. These findings suggest that FGR, even when resolved, may carry clinical significance and warrant attention due to potential perinatal implications. Further research is needed to better understand the clinical relevance and long-term outcomes of resolved FGR.
