(1074) Interpregnancy Interval as a Risk Factor for Pregnancy-Induced Hypertension

Objective:
Hypertensive disorders of pregnancy are a leading cause of maternal and fetal morbidity and mortality. This study aimed to evaluate the relationship between interpregnancy interval (IPI) and risk of maternal pregnancy-induced hypertension (PIH).

Study Design:
We conducted a secondary analysis of patients with one prior vaginal delivery and a subsequent singleton gestation delivered at ≥ 37 weeks between 2004 and 2015. The primary outcome was rate of PIH in the second (G2) pregnancy, categorized by IPI < 2 years, 2-4 years, and > 4 years. Secondary outcomes included individual diagnoses within PIH. Results were further stratified by presence or absence of maternal obesity (BMI ≥ 30 kg/m2) in the G2 pregnancy. Multivariable regression was used to estimate odds ratios for the primary and secondary outcomes and adjust for potential confounders.

Results:
Of the 1435 patients, 478 (33%) had an IPI of < 2 years, 627 (44%) had an IPI of 2-4 years, and 330 (23%) had an IPI of > 4 years. Maternal age and race were significantly different between groups, while the prevalence of chronic hypertension and diabetes was not. Longer IPI was also associated with the development of obesity. Patients with an IPI of > 4 years had a significantly higher odds of PIH compared to those with IPI < 2 years (aOR=2.30; 95% CI 1.42-3.71; Table). A similar trend was seen for gestational hypertension (aOR=2.29; 95% CI 1.28-4.08) as compared to those with an IPI of < 2 years. When stratified by obesity, the increased odds of PIH among those with an IPI of > 4 years remained significant among patients without obesity (aOR=3.19; 95% CI 1.35-7.51) but did not reach significance for those with obesity. However, there was no significant interaction by obesity status (p for interaction = 0.39).

Conclusion:
Longer IPI is associated with increased risk of PIH even after adjusting for maternal age. Although the association appeared stronger among individuals without obesity, maternal obesity did not significantly modify this relationship. These findings suggest that IPI may be an important risk factor for PIH, independent of age and obesity.
