Objective:
Hypertensive disorders of pregnancy (HDP), including gestational hypertension and preeclampsia, are associated with increased long-term cardiovascular disease. While peripartum cardiomyopathy is well-recognized, the link between HDP and subsequent dilated cardiomyopathy (DCM) beyond the peripartum period remains unclear. This study aims to quantify the long-term risk of DCM following HDP and compare risks across HDP subtypes.

Study Design:
A comprehensive literature search was conducted across PubMed, Cochrane Library, Embase and Web of science, to identify studies published up to July 2025. Studies were included if they compared individuals with hypertensive disorders of pregnancy to normotensive controls in the assessment of cardiomyopathy outcomes, including dilated cardiomyopathy. Data was analyzed using a random-effects model and heterogeneity through I² statistics on R software.

Results:
The analysis included three studies with a total of 5.119 patients, of whom 564 (11%) had hypertensive disorders of pregnancy. In the pooled analysis, patients with hypertensive disorders showed a significantly higher incidence of dilated cardiomyopathy compared to normotensive patients (OR 2.42; 95% CI 1.9 to 3.06; p < 0.001; I² = 0%; Figure 1). When evaluating overall cardiomyopathies, the incidence was also significantly higher among those with hypertensive disorders (OR 1.99; 95% CI 1.65 to 2.41; p < 0.001; I² = 72.8%; Figure 2).

Conclusion:
Hypertensive disorders of pregnancy are significantly associated with long-term risk of DCM. This underscores the need for postpartum cardiovascular monitoring and targeted prevention strategies. A history of HDP should be routinely incorporated into cardiovascular risk stratification to guide future cardiac care.
