(189) Social risks-focused lifestyle intervention to reduce preeclampsia in nulliparous Black women: A pilot RCT

Objective:
To evaluate the feasibility and preliminary efficacy of social risks-focused lifestyle intervention on preeclampsia risk in pregnant Black nulliparas.

 

Study Design:
In this randomized controlled trial, pregnant nulliparous non-Hispanic Black women aged ≥18 years and ≤16 weeks’ gestation were randomized 2:1 to receive either the SAIL (Social and Lifestyle) intervention plus routine prenatal care or routine prenatal care (SOC) alone. The SAIL intervention consisted of six monthly in-person group sessions led by a maternal-fetal medicine physician, study nurse, social worker, and perinatal psychologist. Sessions addressed preeclampsia education, stress management, problem-solving skills, and navigation of social resources. Participants completed surveys on preeclampsia knowledge, social needs, perceived stress, and resilience at baseline and in the third trimester. The primary outcome was intervention feasibility. Secondary outcomes included incidence of preeclampsia, live birth, preterm birth, postpartum hemorrhage, SGA, NICU admission,

stress assessed with perceived stress scale, and preeclampsia knowledge assessed with preeclampsia awareness survey.

Results:
A total of 100 participants were enrolled and randomized. Baseline demographics and clinical characteristics were similar between groups (Table 1). Among those assigned to the SAIL intervention, 29 (48%) did not attend any group sessions, including 4 who delivered extremely preterm. Median session attendance was 17% (IQR 0–67%). Five participants were excluded from the analysis due to missing delivery data (n=58 SAIL, n=37 SOC). No significant differences were observed between groups in clinical outcomes (Table 2). However, in the third trimester, the SAIL group demonstrated significantly higher preeclampsia awareness (67% correct responses) compared to SOC (59% correct; p = 0.003).

 

Conclusion:
Although in person group social risks-focused lifestyle intervention was not feasible due to low attendance, it showed potential to improve preeclampsia awareness during pregnancy.
