(659) Clinicians Perceptions of Second Stage Cesarean Delivery Education: A Qualitative Study of Gaps and Opportunities

Objective:
Second stage cesarean delivery is among the most technically challenging and morbid cesarean deliveries, yet educational approaches have remained stagnant. The purpose of this study was to explore clinician perceptions of medical education and training for second stage cesarean delivery management.

Study Design:
Clinicians were invited to participate in a semi-structured interview after completing a national survey on second stage cesarean delivery. Semi-structured interviews were audio recorded, transcribed verbatim, and analyzed using content thematic analysis. Content of the semi-structured interviews was focused on clinician's knowledge and perceptions of second stage cesarean techniques, educations and training, and real-world application in the context of perinatal outcomes.

Results:
A total of (n=41) clinicians participated in semi-structured interviews, representing all four U.S. geographic regions and a range of training levels (resident, fellow, attending) and specialties (e.g. family medicine, general ob/gyns, maternal fetal medicine). Three major themes emerged:  1) Training variability undermines clinician confidence and competency, particularly among both trainees and attendings; 2) Limited access to simulation and hands-on training constraints skill development during and post-residency; 3) Training must be team-based and institution-specific, involving interdisciplinary coordination and multidisciplinary care across obstetric teams. (Table 1). 

Conclusion:
Clinicians identified a critical need for structured, standardized education on second stage cesarean delivery techniques, regardless of training level or specialty. Given its recognition as a high-risk procedure associated with avoidable harm and rising litigation, consistent, multidisciplinary training may better equip clinicians with a reliable surgical toolbox to manage complex second stage scenarios and improve maternal and neonatal outcomes.
