Objective:
Serum albumin is physiologically decreased during pregnancy, but can be further decreased in individuals with preeclampsia due to increased vascular permeability and endothelial dysfunction. Among patients with hypertensive disorders of pregnancy (HDP) without severe features at delivery admission, we aimed to evaluate if hypoalbuminemia is associated with postpartum complications.

Study Design:
This is a retrospective cohort study of pregnant persons aged 18-50 years with singleton term gestations who were diagnosed with HDP without severe features and delivered at two NYC hospitals from 2020 to 2024. Those with chronic hypertension, kidney or liver disease, or missing albumin values were excluded. Eligible individuals were identified via a research database, but data were manually extracted from the medical record. Hypoalbuminemia was defined as < =2.3 g/dL based on prior literature. Study outcomes included development of severe features, magnesium sulfate infusion, antihypertensive use, highest blood pressures, and hospital representation. Patients with low serum albumin were compared to those with values >2.3 g/dL using Fisher’s exact and Chi-square tests with Bonferroni correction for multiple comparisons. Logistic regression was performed.

Results:
Of 572 eligible patients, 231 (40.4%) had hypoalbuminemia during their delivery encounter. Baseline characteristics are shown in Table 1. Patients with hypoalbuminemia were significantly more likely to develop severe features (OR 3.91 95% CI 2.58-5.94), receive antihypertensives during delivery admission (OR 3.60 95% CI 2.16-6.02) and upon discharge (OR 3.01 95% CI 2.13-4.25), and had higher blood pressures postpartum (p< .0001) (Table 2).

Conclusion:
Among patients with HDP without severe features who delivered at term, hypoalbuminemia at admission was associated with the development of severe features and other HDP complications. Albumin levels are readily available in basic metabolic panels routinely collected for HDP and could be used to identify patients who are at increased risk for HDP complications.
