(579) Perinatal outcomes following multifetal pregnancy reduction at a quaternary referral center

Objective:
The management of multiple gestations is complex and may involve multifetal pregnancy reduction (MFPR). Limited data exist regarding procedure-related complications and pregnancy outcomes. Our objective was to describe perinatal outcomes of patients undergoing MFPR. 

Study Design:
Retrospective cohort study of patients with multiple gestations who underwent a MFPR procedure at a quaternary referral center between 2003 and 2024. All procedures were performed by Maternal-Fetal Medicine physicians using potassium chloride injection. Patient characteristics were described, and post-procedure outcomes were compared between early and late reduction. Descriptive statistics [n (%) and median (range)] were used to report findings. 

Results:
Sixty-one patients were included: 30 dichorionic diamniotic twins and 31 triplets (25 trichorionic triamniotic and 6 dichorionic triamniotic). Of the triplets, 23 reduced to twins and 8 reduced to a singleton. The gestational age (GA) at MFPR was 12.7 weeks (11.4-21.0). A trainee was involved in 13 (21.0%); the procedure was technically successful in all cases. The placenta was traversed in 21.3% of cases. Incidental septostomy did not occur in any cases. Among the 30 twins, procedural complications and pregnancy outcomes were reported (Table 1). Post-procedure pregnancy loss occurred in one case. Among those with delivery data available, the interval from MFPR to delivery was 24.6 weeks (16.4-29.0) (Figure 1). The GA at delivery following MFPR was 38.7 weeks (33.1-41.3) for twins reduced to singleton and 34.6 weeks (32.3-37.6) for triplets reduced to twins, and 40.1 weeks (36.4-41.4) for triplets reduced to singletons. Among those with neonatal follow-up data, neonatal intensive care unit admission was 34.4% and overall neonatal survival was 100.0%. 

Conclusion:

At an experienced center, MFPR was technically achievable with favorable pregnancy outcomes and neonatal survival in both twin and triplet gestations.
