Objective:
Patients with Marfan syndrome (MFS) are known to have increased cardiovascular risk in pregnancy. Our objective was to characterize contemporary maternal and obstetric outcomes in pregnancies complicated by MFS using a nationally representative dataset.

Study Design:
This cross-sectional study used the U.S. National Inpatient Sample to identify delivery hospitalizations among patients with MFS from 2016 to 2022. Adjusted logistic regression models were used to compare outcomes between patients with and without MFS, accounting for demographic, clinical, and hospital-level characteristics. Associations were reported as adjusted odds ratios (aORs) with 95% confidence intervals (CIs).

Results:
Among more than 25 million delivery hospitalizations, 1,435 involved patients with MFS. The prevalence of MFS affected deliveries remained stable over the study period [Figure 1]. Patients with MFS were at significantly increased risk of severe maternal morbidity (aOR 5.79, 95% CI 3.29–10.19) and a composite of cardiovascular complications—including aortic dissection or rupture, cardiac arrest, arrhythmia, and heart failure (aOR 7.76, 95% CI 3.61–16.7). MFS was also associated with higher odds of cesarean delivery (aOR 2.19, 95% CI 1.72–2.77) and operative vaginal delivery (aOR 7.89, 95% CI 5.63–11.07). MFS was not associated with hypertensive disorders of pregnancy (aOR 0.69, 95% CI 0.47–1.01). From a fetal perspective, patients with MFS were more likely to experience preterm delivery at both < 37 weeks (aOR 2.19, 95% CI 1.62–2.96) and < 34 weeks (aOR 1.74, 95% CI 1.01–2.97). No significant associations were found with fetal growth restriction or intrauterine fetal demise [Table 1].

Conclusion:
The rate of deliveries affected by MFS did not change significantly over time. Pregnancies complicated by MFS were associated with markedly increased risks of maternal complications, as well as preterm birth. Further research is needed to identify modifiable factors that contribute to these elevated risks.
