Objective:
Gestational diabetes mellitus (GDM) and preeclampsia (PE) are independently associated with adverse pregnancy outcomes, but their combined impact is less understood. This study examines the association between comorbid GDM and PE with adverse pregnancy outcomes.

Study Design:
We conducted a retrospective analysis using the 2017 National Inpatient Sample, including delivery hospitalizations among women aged 12-53 years. ICD-10 codes identified delivery type (vaginal, cesarean section [CS], instrumental), GDM, PE, and outcomes such as preterm prelabor rupture of membranes (PPROM), amniotic fluid disorders, placental disorders, preterm birth, and postpartum hemorrhage (PPH). Adjusted and propensity score-matched logistic regression models were used to calculate odds ratios (ORs) with 95% confidence intervals (CIs).

Results:
Among 592,904 patients, 6.3% had GDM (n = 45,016), 9.6% had PE (n = 68,740), and 1.3% had both (n = 9,150). GDM prevalence was highest in Hispanic (7.5%) and Pacific Islander (9.2%) women, while PE was most common in Black women (13.2%). Independently, GDM was associated with increased odds of PPROM (OR = 1.38; 95% CI = [1.31, 1.46]), amniotic fluid disorders (1.66; [1.58, 1.74]), placental disorders (1.12; [1.06, 1.20]), CS delivery (1.61; [1.58, 1.65]), preterm birth (1.16; [1.10, 1.21]), and PPH (1.06; [1.00, 1.12]). PE was associated with increased odds of CS delivery (1.71; [1.67, 1.74]), preterm birth (1.61; [1.54, 1.69]), and PPH (1.97; [1.89, 2.06]). Comorbid GDM and PE were more strongly associated with certain outcomes, including amniotic fluid disorders (1.92; [1.76, 2.10]), CS delivery (2.81; [2.70, 2.93]), preterm birth (2.20; [2.03, 2.39]), and PPH (1.76; [1.61, 1.94]).

Conclusion:
Comorbid GDM and PE have a synergistic effect on many adverse pregnancy outcomes. These findings support the need for early identification and enhanced clinical monitoring in pregnancies affected by both conditions.
