(1154) Hospital readmission rates for postpartum patients in a remote blood pressure monitoring program

Objective:
Hypertensive disorders of pregnancy (HDP) cause significant postpartum (PP) morbidity. Text-message remote blood pressure (BP) monitoring (RBPM) programs show no difference in readmissions for hypertension (HTN). We aimed to compare readmission rates for HDP patients by engagement with telehealth-based PP RBPM program.

Study Design:
Retrospective study of PP RBPM patients from 11/1/2022-6/30/2024. Eligibility criteria were chronic HTN or HDP. Patients receive BP cuff and scheduled telehealth BP checks, where clinicians follow protocols for BP management. Patients graduate when BP normalizes off therapy or 12 weeks PP. Patients were grouped based on RBPM engagement: 1) declined or enrolled but never attended; 2) attended ≥1 visit but did not graduate; 3) graduated. Primary outcomes were rates of readmission (within 6 weeks PP) for any indication or for HTN. Secondary outcomes were emergency department visits or observations (ED visits) for any indication or HTN. Patient demographics and comorbidites, HTN characteristics, readmissions and ED visits were extracted from the medical record. Groups were compared with chi-square and ANOVA tests and multivariable logistic regression used to determine odds of the outcomes.

Results:
1160 patients were eligible. 508 (43.8%) in Group 1, 195 (16.8%) Group 2, 457 (39.4%) Group 3. HDP diagnoses and magnesium use differed among groups; there were no differences in average mean arterial pressure or rates of stage 2 HTN PP (Table 1). Readmissions occurred in 118 patients (10.2%) for any indication, 91 (7.8%) for HTN. Rates of readmission and ED visits for any indication and HTN all varied significantly between groups (Table 1). After adjusting for confounders, Group 3 had higher odds than Group 1 for readmission for any indication (aOR 3.03, 95% CI 1.90-4.83), HTN (aOR 2.96, 95% CI 1.87-4.76) and ED visit for HTN (aOR 3.58, aOR 1.78-7.17). There was no significant difference between Group 2 and 1 (Table 2).

Conclusion:
Patients fully engaging with a telehealth-based RBPM program are readmitted and visit ED more, suggesting overall increased healthcare engagement PP.
