Objective:
To evaluate the temporal relationship between fetal growth restriction (FGR) diagnosis and the onset of severe preeclampsia (PE).

Study Design:
We conducted a retrospective cohort study of singleton pregnancies complicated by both FGR and severe PE between March 2017 and June 2023 at a single health care system. Pregnancies were included if they had a first-trimester dating ultrasound, a completed anatomy between 18w0d and 24w0d, and were not affected by major structural or genetic abnormalities. The gestational age at FGR diagnosis and severe PE diagnosis was compared, and the sequence of diagnoses was categorized as (1) FGR preceding severe PE, (2) same-day diagnosis, or (3) FGR following severe PE. Comparisons between groups were made using chi-square test for categorical variables and Student’s t-test or Wilcoxon rank-sum test for continuous variables, as appropriate.

Results:
Out of 9386 maternal and neonatal charts and ultrasounds reviewed, 998 patients had FGR, 340 patients had severe PE, and 61 patients had both severe PE and FGR. Among the 61 patients with both, the average gestational age at FGR diagnosis was significantly earlier than that at severe PE diagnosis (30.1 ± 5.1 vs 34.8 ± 3.7 weeks, p< 0.001). Figure 1 depicts gestational age at FGR diagnosis and severe preeclampsia diagnosis for each patient, sorted by the FGR diagnosis timing. Most patients (52/61, 85.2%) were diagnosed with FGR before the onset of severe PE, with a median interval of 29 days (IQR 13–57). Six patients (9.8%) were diagnosed with FGR and severe PE on the same day, and three patients (4.9%) were diagnosed with FGR after the onset of severe PE (two within 2 days and one 22 days later).

Conclusion:
In most pregnancies complicated by both FGR and severe PE, FGR is diagnosed four weeks before the onset of severe PE. These findings suggest that early identification of FGR may offer an opportunity for heightened surveillance and earlier recognition of severe PE. Further research is needed to better understand the pathophysiologic relationship between these entities.
