Objective:
Antenatal corticosteroids (ACS) are administered for pregnancies at risk for preterm birth, such as those with fetal growth restriction (FGR). The optimal therapeutic window is often defined as latency from ACS to delivery of 2-7 days. Yet, timely administration of ACS can often be difficult to predict. The objective was to assess real-world ACS administration practices and evaluate associated factors of timely administration of ACS when given for FGR. The primary outcome was the percentage of patients who received ACS within the therapeutic window. The secondary outcome was characteristic differences amongst the groups.

Study Design:
This was a retrospective cohort study of singleton pregnancies who received ACS for FGR indications which included abnormal umbilical artery Dopplers, severe FGR and new onset FGR at a single institution from 2020 to 2024. Patients with chromosomal abnormalities, fetal anomalies, multiple gestations, or delivery prior to 24 weeks gestation were excluded. ANOVA test and Pearson’s Chi-squared test were used with p < 0.05 to compare characteristics amongst the groups.

Results:
Of 154 patients with FGR and ACS exposure, 7.1% of patients were delivered within the therapeutic window. When incorporating 36 patients who received rescue ACS, 11.7% delivered within the window. Patients who delivered within the therapeutic window from initial ACS exposure were more likely to be multiparous, have history of FGR, given ACS at a later gestational age and for abnormal Dopplers. A subanalysis of those that received rescue ACS was not significant, though limited by sample size.

Conclusion:
We observed low rates of timely administration of ACS for FGR. Yet characteristics such as multiparous, prior history of FGR, later gestational age, and abnormal Dopplers can assist in optimizing timing of ACS. Further studies should evaluate the impact of timing of ACS administration for FGR on neonatal outcomes.
