(649) Geopolitical Variation in MFM Fellowship Website Diversity, Equity, and Inclusion (DEI) Elements

Objective:
To determine if DEI elements represented on MFM fellowship websites differ based on state diversity and legislation.

Study Design:
This was a cross-sectional analysis in July of 2025 of DEI represented on ACGME accredited MFM fellowship websites. Programs that had only initial accreditation and military programs were excluded, leaving 100 evaluable. 11 Diversity Elements (DEs), as defined by Winget et. al (AJOG, 2023), were collected from the individual program websites. To indirectly assess population diversity, state Diversity Index (DI) and Diffusion Score (DS) were collected from the 2020 U.S. Census Bureau data. Higher DI and DS (scale 0-100) scores represent higher population diversity and less concentration in the top three racial/ethnic groups, respectively. The presence of anti-DEI laws (aDEI-L) in each state was collected. Represented DEs were compared according to corresponding DI, DS, and aDEI-L, using Chi-square, Fisher’s exact, Kruskal-Wallis, Mann-Whitney U test, and Spearman’s correlations, with a p-value < 0.05.

Results:
The total number of DEs on websites was not correlated with DI (r=0.095, p=0.35), DS (r=0.121, p=0.23), or aDEI-L (Mdn[IQR]: 4[2,5] vs. 4[3,5], p=0.44). There was no difference in DI scores with any specific DE or with having ≥6 total elements (Figure 1). DS scores were only associated with the DE “Diversity Specific Language” (10.8[7.9, 14.3] vs. 8.6[7.0, 10.8], p=0.015). When compared to states without aDEI-L, states with active aDEI-L had significantly lower representation of “Diversity Specific Language” at 22.2% vs. 47.9% (p=0.02) and “Diversity Inclusion Message” at 14.8% vs. 35.6% (p=0.04). States with aDEI-L had a higher rate of “Diversity Publications” (51.9%) compared to those states without aDEI-L (27.4%, p=0.02) (Table 1).

Conclusion:
Most DEI element representation did not differ among MFM fellowship websites based on state diversity score variations, however the presence of state anti-DEI laws did affect the presence of specific DEI elements.
