Objective:
To evaluate the association between individual- and neighborhood-level social determinants of health (SDOH) and latency (time from diagnosis to delivery) in patients with preeclampsia with severe features managed expectantly.

Study Design:
A retrospective cohort study of patients diagnosed with preeclampsia with severe features between 23w0d and 33w3d from 2016–2025 at a single quaternary care center. Patients were excluded if they delivered within one day of admission or had contraindications for expectant management on presentation (eclampsia, HELLP syndrome, stroke, acute kidney injury, pulmonary edema, intrauterine fetal demise, or placental abruption). Individual-level variables included maternal age, race/ethnicity, insurance type, marital status, and substance use. Neighborhood-level variables were derived from the CDC Social Vulnerability Index (SVI) and dichotomized at the 90th percentile. The primary outcome was latency ≥3 days from diagnosis to delivery.

Results:
Among 405 eligible patients, 269 (66.4%) had a latency ≥3 days, while 136 (33.6%) delivered within 3 days. Demographic and behavioral characteristics were comparable between groups. Both groups demonstrated high neighborhood vulnerability, particularly in the minority status/language domain (median SVI = 0.85). In unadjusted analyses, no individual- or neighborhood-level factors differed significantly between latency groups. Multivariable regression adjusting for demographic, behavioral, and neighborhood characteristics similarly showed no statistically significant associations.

Conclusion:
For patients with expectantly managed preeclampsia with severe features before 34 weeks, SDOH were not associated with latency from diagnosis to delivery. The consistency of this finding across both individual- and neighborhood-level factors suggests that in the setting of severe disease, delivery timing is primarily driven by clinical acuity and obstetric criteria.
